TG/HDL ratio is one of the most powerful and underutilised predictors of insulin resistance and cardiovascular risk from a standard lipid panel.
TG/HDL ratio is one of the most powerful and underutilised predictors of insulin resistance and cardiovascular risk from a standard lipid panel. A high ratio reflects a metabolic pattern dominated by small, dense atherogenic LDL particles even when total LDL appears normal.
Visceral obesity, type 2 diabetes, metabolic syndrome, inactivity, refined-carbohydrate diet, excess alcohol, familial hypertriglyceridaemia. Hypothyroidism and kidney disease also elevate the ratio.
Typically asymptomatic — represents silent cardiovascular and metabolic risk. In extreme TG elevations, abdominal pain from pancreatitis and xanthelasma may appear.
Omega-3 from fatty fish, flaxseed, walnuts raises HDL and lowers TG. Monounsaturated fats (olive oil, avocado). Aerobic exercise is among the most potent HDL-raising interventions.
Sugar, fructose, and refined carbohydrates are the most potent TG elevators. Alcohol strongly raises TG. Trans fats lower HDL while raising TG.
