APRI is a simpler fibrosis risk score using only AST and platelets. It serves as a cross-check to FIB-4: when both scores agree, confidence in the result is high; when they disagree, further assessment (FibroScan or hepatology review) is warranted.
The same drivers as FIB-4: metabolic syndrome, alcohol, viral hepatitis, haemochromatosis, drug-induced injury. Thrombocytopenia from any cause can inflate the score.
