ApoB/LDL ratio is a surrogate for LDL particle size. Each LDL particle carries exactly one ApoB molecule, so a disproportionately high ApoB relative to LDL cholesterol signals a high count of small, dense, strongly atherogenic particles (Pattern B dyslipidaemia).
